Consent to Electronic Records and Signatures
Effective July 22, 2026
By checking the consent box, you agree that we may provide records, disclosures, and documents about your insurance quotes and any policy electronically, and that you can sign them electronically. "We" means New Horizon Insurance Services (CA License #0D01899) and casa.insure, the technology platform operated for it by SimpliSent, Inc.
What this covers
Your quote documents, the surplus lines disclosure (D-1) when it applies, the consumer report authorization, applications, policy documents an insurer allows us to deliver, and the notices this service sends about your request.
Paper copies
You can request a free paper copy of any document at any time: privacy@casa.insure, (602) 476-1003, or New Horizon Insurance Services, c/o casa.insure, 929 N Val Vista Dr, Ste 109 #1110, Gilbert, AZ 85234.
Withdrawing consent
You may withdraw this consent at any time by writing to privacy@casa.insure. Withdrawal does not affect documents already delivered or signed. Because this service operates online, withdrawing may mean we cannot complete your quote or placement online; a licensed agent will work with you on paper instead.
What you need
A current web browser, a device that can display PDF files, an email account you check, and, if you choose text delivery, a phone that receives SMS. If our technical requirements change in a way that risks your access, we will tell you and you may withdraw consent without charge.
