Insurance Privacy Notice

From: New Horizon Insurance Services, California License #0D01899 ("we," "us"), and casa.insure, the technology platform operated for us by SimpliSent, Inc. This notice covers the information we collect when you ask us to quote, place, or service insurance. Our website and marketing data practices are covered separately in our Privacy Policy.
We may collect information about you from others
To prepare your quotes and place your coverage, we collect information from you and from persons other than you. Depending on your application, that includes:
- Claims-history reports about you and the property (for example, C.L.U.E. reports prepared by a consumer reporting agency).
- Property data providers and public records: county assessor and recorder data, aerial and street imagery, prior-listing data, wildfire and other hazard models.
- Insurance companies and their representatives, including prior or current insurers.
- Inspection reports ordered by an insurer after a policy is issued.
What we collect and how
The types of information: your name, date of birth, and contact details; the property's address, characteristics, condition, and safety features; your insurance history (current coverage, lapses, non-renewals, cancellations, declinations); claims and loss history; and the answers you give in our questionnaire. The techniques we use are the questionnaire itself, the third-party reports and data sources above, and documents you upload or send us.
Information obtained from a report prepared by an insurance-support organization may be retained by that organization and disclosed to other persons.
Disclosures we may make without further authorization
California law (Ins. Code 791.13) allows certain disclosures without your authorization. As a general business practice, we disclose information:
- To insurance companies, wholesale brokers, and their representatives so they can quote, underwrite, issue, and service your coverage, including the surplus lines filings that California requires for non-admitted placements.
- To persons performing business functions for us: our technology platform (SimpliSent, Inc), data and claims-history vendors, document delivery and e-signature providers, and communications providers. They may use the information only for the function we hire them to perform.
- To detect or prevent fraud or other criminal activity, or to verify eligibility.
- To insurance regulatory authorities (including the California Department of Insurance) and to law enforcement as permitted or required by law.
- In response to subpoenas, warrants, or other legal process.
- To a lienholder, mortgagee, or lender with a legal interest in the policy, limited to information about its interest.
- For actuarial or research studies, in a form that does not identify you.
- As part of a sale, merger, or transfer of our business, to the participants.
We do not sell this information
We do not sell the information described in this notice, and we do not share it with non-affiliated companies for their own marketing. If that ever changes, California law requires your permission first (Fin. Code 4052.5 et seq.), and we will ask.
Your rights: access, correction, and dispute
- Access (Ins. Code 791.08). Write to us at the contact below and identify yourself. Within 30 business days we will tell you the nature and substance of the recorded personal information we hold about you, let you see and copy it (in person or by mail), tell you who we disclosed it to within the prior 2 years (or who we normally disclose it to), and give you the name and address of any insurance-support organization source so you can contact it.
- Correction (Ins. Code 791.09). You may ask us in writing to correct, amend, or delete recorded personal information. Within 30 business days we will either make the change and notify you, the sources, and the recipients California law specifies, or tell you why we refuse. If we refuse, you may file a concise statement of dispute setting out what you believe is correct and why; we will keep it with the information and give it to anyone reviewing that information.
- Adverse decisions (Ins. Code 791.10). If information about you contributes to a declination or other adverse underwriting decision, we give you the specific reasons in writing, and you may request the supporting items of information.
Retention and destruction
We keep insurance-transaction information while your quote or policy is active and for the period California insurance and surplus lines record-keeping rules require (at least 5 years), then destroy it. Details are in our retention schedule.
