Authorization to Obtain Consumer Reports and Loss History

This is the full text of the authorization you sign when you ask us to quote your home. The questionnaire shows a short version of it above the submit button and links here.
Date: the day you sign. It is filled in automatically, shown to you on the authorization before you sign, and printed on your stored copy.
I authorize New Horizon Insurance Services (California License #0D01899) and the insurance companies and wholesale insurance brokers it works with to obtain information about me and about the property I have asked them to quote.
Who may disclose information under this authorization
Consumer reporting agencies and insurance-support organizations (including providers of C.L.U.E. or comparable claims-history reports), property-data providers, public-records sources, and insurance companies that have issued or quoted coverage for me or for this property.
The information covered
Claims and loss history for me and for the property; prior insurance history (coverage, cancellations, non-renewals, declinations); property characteristics, condition, and hazard data; and public-records information about the property.
The purposes
Quoting, underwriting, placing, and servicing homeowners insurance that I have asked New Horizon Insurance Services to obtain, including the declinations and surplus lines filings California requires.
How long this lasts
One year from the date above, unless a shorter period is required by law. It applies to the quotes and any policy placed from them, and to their renewal within that period.
Your copy
You are entitled to receive a copy of this authorization at any time. Request one at privacy@casa.insure or keep the copy delivered with your quote documents.
Signature
Captured electronically when you check the authorization box in the questionnaire, together with the date above, and stored with your request.
